When it comes to surgery sims, you could go the high-tech, glossy route with Trauma Center, or you could go (very) old school and gritty with Dark Cut 2, a surprisingly well-done Flash game from Armor Games.
You'll probably lose a few patients as you figure out how best to use the limited medical resources available to you. Anesthetic? Not so much; use a shot of whiskey, instead. Actually, use several--your patient is more likely to live through the procedure if he's a bit drunk.
Dark Cut 2 even has Achievements, like "Town Drunk" and "Take the Pain." You'll have to figure out how to earn them on your own, though. That mamby-pamby Dr. Stiles could never handle surgery like this!
